Your First Dental Visit

You’ll be greeted by a member of our front desk staff before handing your new patient forms over to be entered into our system. You’ll then be escorted back to a treatment room where our hygienist will review your medical and health histories as well as take digital images of your oral anatomy. Dr. N. Patel will then come in to perform an oral cancer screening and review digital imaging. Should any problem areas be found, we will create a personalized treatment plan that’s designed to address the issues and ensure a healthier smile.

New Patient Forms

You lead a busy life, and we want to not only provide you with world-class service but also timely appointments that allow you to resume normal activity on your schedule. This is why we’ve made it easy for you to download the forms ahead of time and fill them out at home. All you need to do is make sure to bring them to your appointment completed so that we can get you back into the room as quickly as possible.
